“the ultimate measure of health system performance is whether it helps people recover from an acute illness, live well with a chronic condition, and face the end of life with dignity—and self-reporting by patients is the only way to gauge success.."

Case Studies

Case study 1:

Real-time Insightful Reports

Reports generated from patient data help CNS’s and healthcare professionals gain accurate and valuable insight into a patient’s health and wellbeing. Quick and simple to generate, you can ensure improved efficiencies in clinical appointments and better management of patients.

Your Health Companion collects specific data on:

  • Clinical data or Nurse Assessment Data relating to the disease

  • Quality of life data

  • Patients overall impression of change

  • Disease specific symptoms data

  • General health status

  • Experience of care from health services

  • Cost diary recording expenses connected to patients’ treatment in relation to self-care at home or accessing services

Case Study 2:

Pre-habilitation programmes

Your Health Companion is being used to monitor patients on Pre-habilitation programmes across the country.

On a weekly basis patients are being asked to complete a simple questionnaire that logs their symptoms, and another one that logs the amount of exercise undertaken, weight, alcohol and smoking intake.  By tracking this data and comparing to patients who do not have access to prehab programmes, the teams will be able to evaluate the effectiveness of their programme.

It is hoped that this will help secure recurrent funding for these new services and contribute to the business cases of those hospitals without prehab programmes.

From a patient perspective the app will record their baseline prior to any treatment, this will aid patients to track subtle improvements in their condition since treatment.  This helps motivate and improve compliance with pre and re-hab.

Case study 3:

Patient view

In collaboration with Mesothelioma UK, we have undertaken the Medical Outcomes Research & Evidence (MORE) study in Mesothelioma. This collected prospective cross-sectional data on Patient Reported Outcomes, Experience and clinical information from both patients and Mesothelioma Clinical Nurse Specialists.

MOREin Mesothelioma recruited 503 patients between QI-III 2019. (20% of the prevalent population). Data is available and can be analysed via the Microsoft Dynamics MORE data platform.

Case study 4:

Cost evidence for legal claims

The cost diary enables patients to record all costs connected to a patients’ treatment in relation to self-care at home or accessing services. This can be extremely useful as it provides vital information for mesothelioma patients when making legal claims.

We are finding legal firms are increasingly looking for this factual data that can be made readily available to them to help represent the patients’ case.
